Mixed Martial Arts Training Facility
1. ACCEPTANCE OF TERMS
By signing up for membership, attending classes, or using the facilities at Team Camilo ("the Gym"), you agree to be bound by these Terms and Conditions. These terms constitute a legally binding agreement between you and the Gym.
2. ASSUMPTION OF RISK AND WAIVER OF LIABILITY
PLEASE READ CAREFULLY - THIS AFFECTS YOUR LEGAL RIGHTS
I understand that martial arts training, including but not limited to Brazilian Jiu-Jitsu, kickboxing, mixed martial arts, and related activities, involves inherent risks including but not limited to:
Serious bodily injury, permanent disability, or death
Cuts, bruises, sprains, fractures, and other injuries
Aggravation of pre-existing medical conditions
Contact with other participants and equipment
I voluntarily assume all risks associated with participation and WAIVE, RELEASE, AND DISCHARGE Team Camilo, its owners, instructors, employees, agents, and specifically including Wanderley Camilo Ferreira, Daniel de Vasconcelos Ferreira, Gabriel Ferreira da Conceicao, and Gustavo de Vasconcelos Ferreira, from any and all liability, claims, demands, or causes of action arising from my participation.
3. MEDICAL CLEARANCE
I represent that I am in good physical condition and have no medical conditions that would prevent safe participation. I agree to consult with a physician before beginning training if I have any health concerns.
4. MEMBERSHIP TERMS
4.1 Payment
Membership fees are due monthly in advance
Late payments may result in suspension of privileges
There are no refunds for unused portions of membership
4.2 Cancellation
30-day written notice required for cancellation
Members remain responsible for payment through the notice period
4.3 Freezes
Memberships may be frozen for up to 60 days per year with written notice
Medical freezes require physician documentation
5. GYM RULES AND CONDUCT
5.1 General Rules
Follow all instructor directions and safety guidelines
Maintain proper hygiene and wear appropriate training attire
No shoes on training mats
Clean and disinfect equipment after use
5.2 Prohibited Conduct
Aggressive or disrespectful behavior toward staff or members
Training under the influence of alcohol or drugs
Unauthorized instruction or coaching
Use of facility outside designated hours without permission
5.3 Disciplinary Action
Team Camilo reserves the right to suspend or terminate membership for violations of rules or inappropriate conduct, without refund.
6. FACILITY USE
Training areas are for supervised instruction only unless otherwise authorized
Equipment must be used properly and returned to designated areas
Personal belongings are stored at member's own risk
The Gym is not responsible for lost, stolen, or damaged personal property
7. MINORS
Minors (under 18) must have parent/guardian signature on all agreements
Parent/guardian must remain on premises during minor's training
Special rules may apply for youth programs
8. PHOTOGRAPHY AND VIDEO
Team Camilo may photograph or video record classes and activities for promotional purposes. By participating, you consent to such recordings and their use in marketing materials.
9. LIMITATION OF LIABILITY
TO THE MAXIMUM EXTENT PERMITTED BY FLORIDA LAW, TEAM CAMILO'S LIABILITY FOR ANY CLAIM RELATED TO YOUR MEMBERSHIP OR PARTICIPATION SHALL NOT EXCEED THE AMOUNT OF MEMBERSHIP FEES PAID IN THE 12 MONTHS PRECEDING THE CLAIM.
10. INDEMNIFICATION
You agree to indemnify and hold harmless Team Camilo, its owners, instructors, employees, agents, and specifically including Wanderley Camilo Ferreira, Daniel de Vasconcelos Ferreira, Gabriel Ferreira da Conceicao, and Gustavo de Vasconcelos Ferreira, from any claims, damages, or expenses arising from your participation or breach of these terms.
11. GOVERNING LAW
These terms are governed by Florida law. Any disputes will be resolved in the courts of Brevard County, Florida.
12. MODIFICATIONS
Team Camilo reserves the right to modify these terms with 30-day written notice to members.
13. SEVERABILITY
If any provision of these terms is found unenforceable, the remaining provisions remain in full effect.
ACKNOWLEDGMENT
I have read, understood, and agree to be bound by these Terms and Conditions. I understand this is a legally binding agreement that affects my legal rights.
